Chronic Disease Prevention

Proactive Community Services is tackling community health and economic issues among minority populations in a climate where African Americans are twice as likely as non-Hispanic Whites to die from diabetes and 30 percent more likely to die from heart disease.

Program Objectives

To provide an integrated, seamless model of chronic disease prevention care services, and health promotions by increasing greater collaborations between health care and community settings
To improve chronic disease self-management outcomes through providing the evidence-based Stanford Chronic Disease Program for individuals with one or more chronic diseases
To reduce avoidable hospital admissions and emergency room visits through self-management education
To increase awareness of chronic disease through outreach and preventive education and screenings by hosting or participating in health fairs, workshops, and forums

Health Education Leads to Prevention (HELP)

This program provides a comprehensive approach that includes a strategic mix of health care, health protection, and disease prevention initiatives. The aim is to prevent chronic diseases, enable early detection of disease, and decrease modifiable risk factors combined with encouraging chronic disease self-management.

Community Health Promotion

The goal of community health promotion is to link with public health services, business, social services organizations, including establishing partnerships with hospitals and health care providers to promote healthcare events and activities by making them widely available in disadvantaged communities. A comprehensive mix of initiatives is offered to positively influence individual healthy lifestyle decisions. These initiates information about cancer screenings, the ABCS of heart disease and stroke prevention, blood pressure and cholesterol control, smoking cessation, measures of diabetes control‚ hemoglobin AIC, and obesity‚ body mass index, tobacco use and exposure to secondhand smoke, physical inactivity, excessive alcohol use, diets low in fruits and vegetables, and diets high in sodium and saturated fats.

Health Fairs and Medical and Social Service Provider Forums

Proactive Community Services regularly participate in health events and activities with the aim to improve community health outcomes and encourage dialogue that shares the vision of addressing health disparities to create health equity for disadvantaged, vulnerable populations combined with empowering individuals to take responsible for their health and make healthier lifestyle decisions and finding ways for community-based organizations, public health departments, businesses and governmental agencies to work together support efforts and community programs that link individuals with resources.

Health Education and Risk Reduction

The goal of health education adn risk reduction is to achieve and sustain healthy people and communities throughout south suburban Cook County. Chronic disease prevention interventions and initiatives are provided to promote healthy lifestyles, increase chronic disease education, and reduce instances of chronic diseases such as heart disease, hypertension, cancers, diabetes, asthma, HIV/AIDS/Sexually transmitted infections, Hepatitis C, and other chronic diseases associated with widespread premature death.

Our Impact

During the last 15 years, PCS prevention services have reached over 15,000 individuals at high-risk for acquiring or transmitting HIV/STI Infection through workshops, community outreach activities, presentations, events, and literature and safer sex package distribution. HIV/STD counseling, testing, and referral: 97% of our clients say that they felt empowered to change their risky behavior and they would refer others to our program.
